THE CONCIERGE PROGRAM DETAILS

 

Laurie is taking only 150 patients in her Concierge primary care practice.

 

COST: $2000 annually

 

Insurance Accepted- Medicare and most non- HMO insurances. If you do not have insurance or you have an HMO, there will be a $200 fee for each face to face visit.

 

 

FOR $2000 A YEAR, YOU WILL RECEIVE THE BEST PRIMARY CARE THAT ANY LOCAL PHYSICIAN CAN OFFER.

 

This fee will give you the special services that Medicare or your insurance carrier will not pay for.

 

  • Available 24/7 via cell phone

  • Privileges at Martin Memorial Hospital; coordinates quicker ER visits, and daily hospital visits (unless on vacation or sick). You will not be stuck with a Hospitalist who is not familiar with your medical needs.

  • Coordinates with out of town family 

  • Receive a yearly (1hr) detailed full head to toe annual medical exam with treatment plan & goals.

  • Meets you in the ER 

  • Coordinates all hospital discharges and makes sure you return home safely with proper follow ups and medications

  • If needed, prescriptions will be organized weekly and delivered to your home at no charge.

  • Coordinates all specialist visits

  • Keeps all your medical records from all providers on a thumb drive that will be updated with each visit.

  • Advises on what rehab to chose if one is needed

  • Home visits or visits in any location in Martin County, or her office.

  • Assists with end of life care decisions

  • Advises on and helps with Assisted living placement, nursing home placement

  • Advises on Dementia treatment and care

  • She will be your care manager and ensure every specialists communicates with one other. (extra cost)

  • Attends specialist visits if needed (extra cost)

  • Arranges for transportation anywhere you need to go (extra costs)

  • She will visit in rehabs to ensure clients goals are being met. (extra charge)

  • If you don’t have time to be seen, she will triage your needs over the phone or email and prescribe medications or tests as needed. She will be able to oversee your care even when you are away.

  • Discusses all diagnostic or lab test results with you in person or over the phone.

  • SHE WILL BE THERE FOR YOU!

 

Services than can be done in the home or office under your insurance program or fee for service may include:

 

Blood work

X-rays

Ultrasounds

Stitches

EKG

Vaccines

Wound care

Iv antibiotics

Cardiac monitoring

Pulmonary treatments

Psych therapy

Physical therapy

Occupational therapy

Speech therapy

 

She is so confident that you will be happy with her services, she is willing to give a full refund within 90 days for any client that is not 100% satisfied.

 

All clients must have Medicare or a PPO insurance.

Or, get everything listed above but pay $200 per visit needed.
